PCSC Provider Toolkit
The PCSC Program consists of viewing virtual learning collaborative sessions, in addition to submitting Plan-Do-Study-Act (PDSA) worksheets unique to each of the 3 modules. Specialty care practices learn concepts through instructional webinars, practice these concepts using a PDSA cycle, and then reinforce the concepts by completing PDSA worksheets. Learning Collaborative Curriculum 
The first module will focus on the implementation of care compacts, by the specialty practice to promote effective communication and improve care coordination within the medical neighborhood. Objectives of this module will be to understand the components of a care compact, define a good referral, and establish practice specific standards for elements within the compact to support a collaborative approach to providing comprehensive patient care  
The second module focuses on specific actions that can be taken to improve the quality of care to patients and streamlining processes to improve HEDIS measure compliance. Objectives of this module focus on implementing population health management strategies with an emphasis on tracking and outreach, patient education and identifying methods to improve clinical documentation  
The third module will focus on impacting the efficiency of care to actualize the Triple AimĀ© of improved quality, increased patient satisfaction and lower healthcare costs. Objectives include recognizing what cost of care means to stakeholders in the healthcare system and identifying cost effective, high quality solutions in healthcare choices that can lead to decreases cost, reducing waste and overuse  
Continuing Education  
As of January 1, 2018, CEU and CME credits will be discontinued.
