Patient-Centered Specialty Care Program Pilot
The Patient-Centered Specialty Care (PCSC) program pilot is aimed at continuing the model of support for primary care focused initiatives by adopting the principles of “The Patient-Centered Medical-Home Neighbor: The Interface of the Patient-Centered Medical Home with Specialty/Subspecialty Practices” , as published by the American College of Physicians (ACP) in 2010. ACP’s concept of a Patient-Centered Medical-Home Neighbor strives to define the role of specialists within the broader medical neighborhood via four main principles: 
Effective two-way communication between primary and secondary providers
Appropriate and timely referrals and consultations with prompt feedback of findings / recommendations
Effective co-management of patients when necessary
A commitment to practice in a patient centered fashion across all physicians delivering care to a patient
These four principles shape our PCSC program and foster a network of high value specialists to participate in the broader patient centered care team and support primary care physicians in the overall management of their patients. 
PCSC Program Support  
The specialty practice is self-guided through this process with the support of quality improvement virtual tools/resources to facilitate practices in the journey towards their “dream” referral process. Available supports to practices include:
