Lumenos Plans for Group Business

Recognizing that fundamental change is necessary to rein in health care costs, we're taking action by changing the way you and your customers view health care. 
Our family of AnthemĀ® Lumenos plans focuses on longer-term solutions to health care, which do more than simply pay for care when a member is sick. These plans manage costs by giving people more control over their health care dollars and offering incentives to make healthy lifestyle changes. Health information, support tools, incentives and financial accounts create informed, engaged consumers who make better health care decisions that help control health care costs.  
We offer four types of Lumenos plans. The difference is how the health account is funded. Click on any of the links for additional product information. 
Health Reimbursement Account (HRA) where the account is funded by the employer.
Health Savings Account (HSA) where the account can be funded by both the employer and the consumer with tax advantages.
Health Incentive Account (HIA) where consumers earn dollars for the account as a reward for engaging in healthy behaviors.
Health Incentive Account Plus works just like the HRA, but the account is funded by us, not the employer or member.
 
These Lumenos products are available for quoting today to your fully-insured clients, with effective dates beginning October 1, 2008. 
 
Please Note:  
The turnaround time for the successful implementation of a High Deductible Health Plan can be in excess of 30 days in preparation for the desired effective date. In the event that all forms are not completed and received within this timeframe, the client will experience delays in processing, group loading and distribution of ID Cards. In cases of clients selecting a High Deductible Health Plan (regardless of whether a case is new or renewing), our consumers will need to complete an Employee Enrollment Application in its entirety as part of the process, in addition to any other plan-specific documents. Completing an Employee Enrollment Application as part of a client’s renewal, will replace the current process of completing a Member Change Form. Any missing information and/or documentation could result in the effective date being moved to the next available date.
