You must have an active license in life, accident and health in order to apply.
A copy of your E&O declarations page is required.
All applications for appointment undergo a comprehensive screening process.
All agents applying for appointment or reinstatement must pay a $90 Appointment Fee (checks made payable to BCBSGa).
