Before you can start representing BCBSGA and/or its affiliates, you must be licensed by the Georgia Department of Insurance.
You must have an active license in life, accident and health in order to apply.
A copy of your E&O declarations page is required.
All applications for appointment undergo a comprehensive screening process.
All agents applying for appointment or reinstatement must pay a $90 Appointment Fee (checks made payable to BCBSGa).
All forms listed below are required to be completed.
 
New appointed agents interested in selling individual policies should contact their Regional Sales Executive for more information and upcoming classes.
